Dr Wee Xue Ting, Novel Pre-Consultation Pharmacist Services and Collaborative Prescribing in Primary Care in Singapore
Primary care systems across Asia remain largely physician-centric, with pharmacists traditionally limited to dispensing and advisory roles. At the same time, ageing populations, rising multimorbidity, and constrained physician capacity are placing increasing strain on healthcare systems, particularly in larger settings where timely access to care is challenging. In response, this presentation highlights two complementary pharmacy practice innovations implemented through the iRx framework in Singapore. A key component is the integration of pharmacist-led pre-consultation interviews, which reposition pharmacist involvement to before the physician encounter. By conducting structured assessments ahead of the consultation, pharmacists identify drug-related problems, optimise medication regimens, address adherence barriers, and better prepare patients for focused, higher-value physician interactions. This shift in timing redefines how pharmacists contribute within routine primary care workflows. Alongside this, a collaborative prescribing model enables pharmacists to work under formal practice agreements with physicians to initiate, adjust, and monitor therapy. While such models are increasingly established in Western healthcare systems, they remain rare across Asia. Redistributing aspects of medication management to trained pharmacists offers a practical and scalable approach to extending primary care capacity while maintaining quality and safety. This session will present early clinical outcomes demonstrating meaningful improvements in glycaemic control, blood pressure, and cholesterol levels, alongside real-world implementation insights. Attendees will gain a practical blueprint for embedding pharmacists into primary care teams as active partners in care delivery, including key enablers, challenges, and lessons to support adaptation across diverse healthcare settings.

Ms Too Hui Ling, Community Pharmacists’ Role in Population Health
Community pharmacists are uniquely positioned to play a pivotal role in the early detection and management of Cardiovascular Disease (CVD) risk factors. The presentation explores the implementation of point-of-care (POC) testing services within Community Pharmacy settings to monitor key indicators of cardiovascular health. With their accessibility and trusted relationships, pharmacists can deliver convenient and comprehensive health assessments, including POC testing for HbA1c, Cholesterol levels, and Blood Pressure—critical markers for identifying individuals at risk of CVD. By bringing these services closer to the community, pharmacists can detect newly diagnosed or poorly controlled conditions, enabling timely intervention and ongoing disease management. As part of this initiative, we piloted the Decentralized Health T.A.G. (Test, Advice, Go!) service with Guardian. This service streamlines the patient journey by guiding individuals through testing, providing personalised advice, and connecting them to appropriate follow-up care. Building on this, an upcoming partnership with Omron and the Pharmaceutical Society of Singapore (PSS) will introduce blood pressure and atrial fibrillation screening across 30 community pharmacies, further expanding access to care. Evidence shows that community-based services and pharmacist-led interventions improve clinical outcomes, including glycaemic control, lipid management, and blood pressure regulation. These services also enhance medication adherence and patient education. The presentation will outline the clinical, operational, and logistical considerations for implementing POC testing services. Expanding the role of pharmacists in CVD prevention can significantly reduce the burden of chronic disease.
